Attendant Care via Community Developmental Disability Program (CDDP)
Attendant Care Services
Attendant Care Services include direct support provided to an individual in the home of the individual or community by a qualified personal support worker or provider organization. ADL and IADL services provided through attendant care must support the individual to live as independently as possible and be based on the identified goals, preferences, and needs of the individual.
Employment Services
Employment Services include job coaching services through the CDDP and via contracts with the Oregon Commission for the Blind and Vocational Rehab (VR). We also hold a VR contract to provide job placement/development services to individuals in Track 1, 2, and 3.
Professional Behavior Services
We provide Professional Behavior Services to individuals with challenging behaviors that impact their ability to perform ADLs and IADLs.
Providing support and personal assistance for people with a developmental disability is a core service option with HDE Home Care. Our view is the basic tenant of humanity is providing care for all, especially those who have unique needs.
Our direct support staff can provide a range of services custom tailored to your loved one's individual needs, including community transportation, professional behavior services, attendant care, and employment services. However, it is the human services element that we feel matter the most, connecting with the person and provide life-enriching comfort and service.
Our team works with the individual, any natural supports, and case management to understand the individualized support plan. Prior to beginning services through the CDDP, we meet with the individual to interview and get to know the individual's care needs and life goals. Upon the start of services, we develop a service plan to help communicate instructions to direct care staff.
Community Transportation Services are available when natural supports or volunteer services are not available. We provide community transportation to access allowable goods and services, as outlined in the individualized support plan.
